  • Photos and shade information – Include patient photos (smile, retracted, profile) to help guide esthetic contouring.
  • Treatment goals – Notes on tooth positioning, changes in length or shape, vertical changes, or planned surgical procedures.
  • Restoration type – Specify whether you’re planning veneers, dental crowns, bridges, or implant-supported restorations.
