According to the American Dental Association, roughly 5 million implants are placed each year. The use of implant therapies to restore missing dentition has grown considerably over the last several years.

Implants offer a superior restorative option for most patients. The increased popularity of implants in treating edentulism has led to explosive growth in material options, processes, and techniques in achieving successful results. One key area in this success is laboratory fabricated provisionals in implant cases.

One crucial part of successful implant cases lies in the proper temporization of the case. Provisional restorations are tasked with maintaining adequate function and maintaining or improving the esthetics of a smile until the permanent restorations are completed. They keep the surrounding teeth from moving, including keeping opposing teeth from extruding, and they provide a marker for soft-tissue management.

Also, in treatment planning, they play a crucial role in identifying the best design for the case.

Once inserted, the patient can then evaluate and provide feedback to ensure the final restorations meet the patient's needs.

One of the breakthroughs in temporization in dentistry is Burbank Dental Lab's proprietary SMART 1 implant temporaries. SMART 1provisionals are a polymethyl methacrylate (PMMA) material. The material used in SMART 1 Implant Provisionals, coupled with our laboratory fabrication process, makes these temporaries more durable than chairside fabricated provisionals.

As a result, SMART 1 provisionals are recommended for long-term temporization, making them ideal for a wide range of case needs, including more significant complex cases and implants. SMART 1 provisionals are easy to adjust. Also, they can be highly polished, helping to minimize plaque accumulation, which aids in helping to control tissue health.

Cement-Retained vs. Screw-Retained

The SMART 1 provisional process can either be used on cement-retained or screw-retained implant cases.

In cement-retained restorations, the most significant advantage is that they are much easier to fabricate than screw-retained restorations.

The downside in choosing cement-retained implant restorations is that they can often come loose due to de-cementing. This issue can result in the need to handle the abutment and the tissue while they are still healing.

One of the most significant disadvantages is the possibility of trapping cement in soft tissues. Cement entrapment can result in loss of bone and, ultimately, failure of the implant.

In comparison, screw-retained restorations eliminate the need for cement, thereby removing the possibility of bone loss directly at the site.

Screw-retained restorations are more challenging to fabricate because of having to deal with the access hole. With upper anterior implants, it is possible to have the access hole coming out on the facial or incisal of the restoration, which is unacceptable in most cases.

Implant Temporization Workflow

Technology has made vast improvements to both clinical procedures and laboratory manufacturing processes.

Dental offices have made advancements with the introduction of cone beam computerized tomography (CBCT) (see above image)and digital impression methods. Laboratories have also benefited from scanning technologies, computer-aided design, computer-aided manufacturing (CAD/CAM)(see below image), and 3D printing.

Incorporating these technologies can significantly enhance the patient experience, improve outcomes, and improve profits. The digital workflow process is outlined below.

First, the clinician takes a CBCT scan and a digital scan of the patient. This data is sent to the lab where it is uploaded. Here, both the CBCT data and digital scan data are combined and queued to begin the treatment plan. Traditional impression methods can be used. With this method, the laboratory will pour the models, and then these models will be scanned to convert the information into digital data.
The data is used to fabricate a surgical guide (See figure below).
A screw-retained or cement-retained restoration is designed using design software.
Once the design is complete, the restoration(s) is milled.
The appropriate guide rings are inserted into the surgical guide and evaluated on the model to check for retention.
The SMART 1 crown is evaluated to ensure the proper fit and contact has been achieved.
A seating jig is sent along to ensure proper alignment.
SMART 1 provisionals can be adjusted if necessary.

Once all of the implants have successfully osseointegrated, the final completed prosthesis can be fabricated.

Duplicate the SMART 1 Provisional

For screw-retained cases, it is possible to create a duplicate SMART 1 provisional.

This extra temporary is beneficial when adjustments have been made to the provisionals throughout the patient's healing process.

As the patient wears the provisionals, they can communicate any necessary changes, and in most cases, the changes can be made by the clinician chairside.

By the time the final restorations are ready to be fabricated, the patient and clinician have worked out and agreed to the definitive prosthesis.

This final adjusted provisional can be sent back to the lab to be duplicated in the final restorations. During this time, the patient will wear the duplicate temporarily until they receive the final restorations.

There are vital areas to consider to achieve consistent success with implant cases. Aside from the physiologic aspects such as bone considerations, placement of the implants is critical for case success.

Also, adequately designed provisional restorations using highly polishable materials such as Smart1 that act as placeholders during the healing process play a crucial role in achieving an optimal outcome.
