One of the areas of interest that we get many inquiries about are questions that relate to what prosthetic options are available for the restorative phase of treatment. Specifically, what options are available that can be built on the guided surgery plan. The questions include inquiries about custom healing abutments, and desires to have a provisional or even a final restoration at the time of surgery.

One of our significant partnerships in the area of guided surgery planning is with Blue Sky Bio (BSB). A key reason we work very closely with BSB is due to the open nature of Blue Sky Plan. This software is quite versatile and can allow both lab and clinicians to enter into the digital planning ballgame in several domains of dental treatment, including implant surgical planning, orthodontics, and most recently digital denture design.

This article will only address implant surgical plans and restorations that can be facilitated by the use of Blue Sky Bio Plan. It is not the only software option that can make this whole package available, but it is the specific one that will be addressed in this article.

Many clinicians use Blue Sky Plan to facilitate their implant surgical plans and design their own guided surgery appliances, and then send the files to Burbank Dental Lab to fabricate the appliance.

Burbank Dental Lab also offers surgical planning that is overseen by a dental clinician. This process allows the clinician to delegate the activity of planning the surgery to Burbank Dental Lab’s team, but still empowers you the clinician, to approve the proposed plan, and gives you access to interface with a peer when needed to adjust your plan prior to the guide fabrication.

If you do your surgical plan and guide with Burbank Dental Lab, it allows for additional benefits that extend beyond the guided surgery appliance. You can also request a custom healing abutment  (Figure 3) or a screw-retained provisional.

Note: Both provisionals must be made as titanium cylinder with a PMMA coronal portion that must be bonded together in the mouth. (Figure 4)

Bonding the 2 pieces together in the mouth compensates for variations in the actual placement of the implant.

One of the most critical aspects that contribute to the long term success of an implant will rely on proper tissue contouring. A guided surgery workflow can facilitate the fabrication of a custom healing abutment or other provisional that custom designs the emergence off of the implant. This is done to create an ideal shaping of the tissue at the initial surgical appointment.

When the tissue is supported by an anatomical custom healing abutment, there exists the opportunity to keep and create the interproximal papilla maximized, which will minimize any interproximal gaps that can be problematic both esthetically and hygienically.
