Choosing the most suitable material for a smile restoration is crucial to success in dentistry.

A dental veneer case must be evaluated based on three primary characteristics: esthetics, durability, and adaptability.

Dentists, dental labs, and patients must collaborate to make dental veneer cases successful through mockups, communication, and proper treatment planning. To guarantee a successful smile restoration, the dental lab and dentist must understand which materials will work best for a specific case, taking into consideration the patient’s dental history, and the existing dentition. Understanding all of these pieces and creating the proper treatment plan will guarantee a positive outcome and leave the patient with a natural, beautiful, and long-lasting smile.

A Case Study

The following case was sent to Burbank Dental Lab by Dr. Sergey Zakharov of Family Dentistry Riverside Crossing in Greer, South Carolina.

The patient was a 30-year-old person who presented with composite dental veneers that he had been placed over ten (10) years ago while he was in Moscow, Russia. The dental veneers had since become discolored and aesthetically unpleasing. In addition, tooth number 14 had received endo treatment which failed.

The patient’s original dental veneers were placed to help correct his enamel hypoplasia.

Patient’s Goal

The patient’s current chief complaint was that his porcelain veneers were dull, and he wished to brighten up his smile and improve the overall aesthetics of his teeth.

It was determined that this case could be beautifully restored with ¾ dental crowns on 4-11 and a full crown on number 12. Tooth #14 would be restored with a bridge from 13 to 15.

Preparation

Before preparing the patient, a digital scan of the patient’s current dentition was performed using an Itero intraoral scanner.

A wax-up was created to incorporate the ideal length, shape, and function of the proposed restorations. A mockup was created and delivered to the patient for a try-in.

The patient approved the mockup, and the case was prepped and made ready for the final restorations.

This step is a great way to ensure the following:

  • The mockup provides a template and preview of the final outcome.
  • The patient evaluates the length, shape, embrasures, and midline and expresses any desired changes at this time.
  • The clinician can check for any speech issues, such as lisps or whistling.
  • Occlusion issues can also be pre-evaluated and addressed before the final restorations are fabricated.

After evaluating the mockup, the patient agreed to proceed with the restorative dental work.

Material Selection

To restore the anterior units 4-11 and create symmetry and a brighter, whiter smile, ¾ E.Max layered porcelain veneers were determined to be the best option.

To restore tooth number 12, ZIRMAX M was determined to be the best option. Additionally, it was agreed that a ZIRMAX M bridge would be the best option to restore 13-15.

The patient was then prepared, and digital impressions were taken. The shade was selected to create a whiter and brighter smile .

The digital files were sent to Burbank Dental Lab to begin the case fabrication process.

The case was already worked out and approved in the mockup phase. Having the approval from the patient through the mockup helps take any guesswork out of the final fabrication process and provides the patient with a predictable familiar outcome.

The E.Max veneers were ready to be pressed, and the ZIRMAX M restorations were digitally designed and milled. An expert dental technician finished the restorations, beautifully blending the E.max veneers with the ZIRMAX M dental restorations.

After the restorations were inserted, Dr. Zakharov took an additional digital scan that was sent to Burbank Dental Lab to fabricate a maxillary hard/soft night guard.
