BiteSoft: Protecting Against Bruxism

Bruxism is defined as excessive teeth grinding and clenching of the jaw. It is a common parafunctional habit and is a prevalent issue among the population. People suffering from bruxism continuously clench and grind teeth, either during the day or at night, causing tooth wear, face and jaw pain, tension headaches, temporomandibular disorders, and even damage to restorative work.

Bruxism is a subconscious neuromuscular issue and is estimated to affect one-third of the people in North America. It is said to have multiple causes, including central factors, peripheral factors, and psychosocial factors. It is essential that these issues are properly diagnosed and managed to prevent damage. A bruxism appliance can be vital in protecting dentition and relieving pain.

One of the best treatment options for a person suffering from bruxism is the custom-fitted BiteSoft® splints (Figure 1, shown below).

They are worn over the anterior teeth from canine to canine and provide a pain-free and safe way to manage the harmful effects of bruxism. They are indicated for the maxillary arch and are used while sleeping.

The BiteSoft design includes a lingual ramp that provides mandibular stability and helps to relax tense jaw muscles. It is fabricated using a dual laminate that consists of a soft inner surface and a rigid outer surface. Custom made, these night guards are designed from the patient's exact bite and vertical dimension of occlusion to offer the most accurate fit, resulting in a high degree of comfort.

Symptoms of Bruxism

When prescribed, BiteSoft is an effective solution to providing relief from the damaging effects of bruxism's clenching and grinding. It helps establish relaxation and stability in the jaw through its lingual discluding ramp and centric occlusion table. Some common symptoms associated with a patient suffering from bruxism include:

  • The patient has been told they grind their teeth at night.
  • They experience prevalent headaches and/or jaw pain.
  • Wear and fractures are visible on natural teeth and dental restorations.
  • The jaw makes a clicking sound.
  • The patient complains of poor sleep and feeling tired.
  • There is an abnormal alignment of the teeth.
  • The patient's teeth are sensitive to heat and cold.
  • The teeth have become loose.
  • The patient experiences earaches.

One of the most significant advantages of BiteSoft is that it can be used to treat and protect several dentofacial issues so that only one type of appliance is necessary to cover a wide variety of bruxism related issues. Overall, BiteSoft reduces the intensity of bruxism by 70%, helping to significantly relieve symptoms. It can be used to prevent tooth damage, protect restorations, relieve jaw pain, and neck pain.

The appliance is fabricated with a hard polymer outer surface and a softer material on the inside. This dual-material approach provides protection, especially for porcelain restorations. Hard acrylic appliances can put stress on porcelain restorations, causing cracks, fractures, or crown loosening. The soft inner surface protects against this type of damage and helps patients protect their dental investment, and improves the integrity of the clinicians' dental work. The benefits of using a BiteSoft appliance are as follows:

To fabricate an appliance, the clinician must take a Polyvinyl Siloxane (PVS) impression of both the mandibular and maxillary arches. (fig.2, shown above) This information is sent to the lab to fabricate the final appliance. Special bite registrations are not required.

Delivery Instructions

Once the appliance is fabricated, the appliance should be evaluated. To do this, insert the appliance in the patient's mouth and evaluate to (fig.3, shown below) ensure that it cannot be removed easily with tongue or lip pressure. The patient should not be able to retrude the mandible beyond the posterior edge of the ramp.

The best way to check this is to make sure the patient is lying down. The lingual ramp guides the laterals onto the incisal table. To optimize bruxism inhibition, the occlusal load is evenly distributed over 3 to 4 opposing incisors. It is essential to check that the incisal table is at right angles to the long axis of the opposing anterior teeth. Posterior teeth require minimal clearance, so the appliance's vertical opening does not need to be more than two millimeters.

Compliance is crucial for a BiteSoft appliance to be effective. Often these appliances take time for the patient to get used to wearing. In the beginning, some patients experience muscle tenderness and discomfort.

To minimize this, patients should be instructed to wear the BiteSoft appliance for a few hours overnight until they begin to adapt and become more comfortable.

BiteSoft is one of the most successful appliances in treating Bruxism and easily receives the most positive feedback. With a properly-fabricated BiteSoft appliance, patients will sleep better, be relieved of pain, and minimize damage to natural teeth or dental restorations.
